Each set of photos must include at least one picture of the front of the scalp and one of the top to show the full extent of your hair loss. You should try to take these photos under the same conditions for objectivity.

These photos must be good quality and allow us to clearly see your condition. The photos need to be dated, either automatically by the camera or by the developing service. For verification purposes, please include a photocopy of your driver's license or other picture identification, which gives your age, such as your passport.
